A link to KPS Enrollment Forms is provided to the left. Please complete each of the required forms: Registration, FERPA, Immunization and Physical. If you are coming from another school you may also fill out the Request for Records Release form. You may email the forms or print them but you must still register in person at the Enrollment Office. Please help us prepare for your child's arrival by completing the Registration Survey below.
Items you need to bring with you when you register:
- Proof of Residency in the Kearney Public School district. Your name and address must be on the documents you provide.
Examples: Rent receipt, lease agreement, mortgage document, property tax record, voter registration record, current utility bill, government benefits documentation such as medicare, disability or food stamps.
- Immunization Records for your child.
- Official Birth Certificate. This needs to have the raised seal. (A copy will be made for our files)
- Physical Exam Form. This is required for students entering Kindergarten, 7th grade or new to Nebraska. Pick up a form at our office or use a form provided by your family Doctor. The form must be signed by your Physician. Students who enter formal schooling as first graders also require physicals.
- Student Record or Cumulative File from school last attended. The best practice is to have the former school send the file to the new KPS school site: Whittier Learning Resource Center, Attn: Enrollment Office, 310 W 24th Street, Kearney NE 68845. You may call the former school and make this request or sign a Request for Records form.
