This form is required for any NEW Voluntary Transfer requests, that is, if you want your child/ren to attend a school other than the school they are assigned. If you have Existing Voluntary Transfer students you will be given a different form to renew their transfer requests
Middle School Voluntary Transfer Form
This form is required if you do not wish to attend the assigned Middle School.
